SERVICE AGREEMENT

Service Agreement

This Service Agreement ("Agreement") is entered into between Dr. El Family Advocacy Services ("Service Provider") and the client ("Client") effective as of the date the Client consents and pays for services.

Services Provided:

Service Provider agrees to provide family advocacy support to Client, including but not limited to consultation, guidance, and the development of a Parenting Action Plan.

Initial Consultation:

Client agrees to participate in an initial consultation meeting with Service Provider, lasting up to one hour. During this meeting, Service Provider will gather information about Client's needs, goals, and concerns regarding advocacy for their child.

Parenting Action Plan:

Within 48 hours following the conclusion of the initial consultation meeting, Service Provider will deliver a Parenting Action Plan to Client. This plan will outline specific strategies, recommendations, and action steps for Client to implement in advocating for their child.

Client Responsibilities:

Client agrees to work diligently to implement the strategies outlined in the Parenting Action Plan. Client acknowledges that they are implementing the plan voluntarily and with their own consent. Client understands that Service Provider is offering subject matter expertise and support but does not guarantee any specific outcomes in relation to third parties, such as educational institutions or service providers.

 

Family Coaching Program:

Following the initial consultation, Client may choose to sign on for Service Provider's Family Coaching Program, which consists of a minimum of five sessions. The terms and pricing of the Family Coaching Program will be discussed and agreed upon separately.

 

Fees:

The fee for the initial consultation and Parenting Action Plan is $250. Payment serves as agreement to the terms of this Agreement. No refunds are offered for services rendered or for missed initial consultation meetings. A 50% cancellation fee applies for any cancellations made up to 24 hours before the initial consultation meeting.

 

By selecting "Yes, I agree" and submitting payment, the Client acknowledges that they have read and understand the terms of this Agreement and agree to be bound by its terms and conditions.
